Primary Care includes the following:
- Annual wellness visits

- "Quick-Care" visits- usually same day, next day for acute illness/injury
- Prescription refills
- Preventative care (vaccinations, screenings, lab monitoring, etc.)
- injuries & minor procedures (cuts, incision & draining, woundcare, trigger point injection, etc.)
- Integrated physical and mental health services- counselors on site
- Referral to specialists when needed
- Home Health and durable medical equipment (DME) orders
Here's how Telemedicine works:
This is an excellent way to remain safe at home while getting your healthcare needs met.
Once you have completed online intake information, you can call or self-schedule through the patient portal where you'll have access to your healthcare record. Don't worry, we'll walk you through the process.
You will receive an email or text (your preference), with a link to "enter the waiting room" at the time of your appointment. It is really just that simple!
At your appointment time, you'll be connected with your provider for your visit.
Prescriptions &/or orders are sent electronically to your pharmacy, lab, etc.
Other Services:
We are able to send a nurse practitioner to see patients who have difficulty going out to see their healthcare provider if they live within 20 miles of our office. Some examples are patients returning home from hospital or rehab, live in assisted living, or are unable to leave home easily. We work closely with home health agencies to ensure our patients are receiving the best possible healthcare in their home.
